Causes for severe draws or hooks

I know it is tough to diagnose without video......

my ball is taking off with velocity at a lower angle and ends up drawing severely or hooks...

Only thing i can think of is my hands are way ahead of the club face and i am perhaps bringing my clubhead to far inside on my first move..

any ideas?
