You get to the top very nicely on plane. Your problem appears in the transition, you start down with your upper body and this gets the club very steep early in the downswing, it then moves outside the ball forcing you to re-root it cutting back across.
You need to work on starting the downswing with your lower body so that the club drops down into the slot, you will be able to then come more from the inside with a shallower angle of attack.
The 'Pump Drill' is a great way to get the correct feel in the downswing. See this video:

Look at your hands. Based on the video it appears that when you take the club back and you begin to cock the hands you will be able to see the logo on your gloves, it will be facing you. That is no good. When you cock your hands on the up swing the logo should face out away from you.
It also appears that you are cocking the hands to early, almost as soon as you take away the club.

This is actually one of the more rare type of OTT moves a person makes.
Generally I find 3 reasons a person swing steep on their forward swing.
1) A person leans forward toward their target to start their forward swing.
2) A person starts their swing by pulling with the left shoulder instead of dropping the right shoulder down and beginning with their lower body.
3) A person counter-rotates their forearms. (Counter rotation causes accross the target line at the top)
You have #3 going on but it's at the VERY last second. Essentially it looks like to start the swing going forward you are pulling down with the right arm and pulling it close to your body.
Right now you need to make yourself aware of this action. Not just seeing it on video, but feeling it happen. So next time you are out swinging the golf club just try to rate the "tension" in your right arm at the start of the forward swing. Rating it 1-10.
