any tips on helping the real swing look more like the practice swing. I can get my backswing into a great position in the practice swing but when I do the real thing I immediately feel it go to the inside, and way too flat. I need to get this subconcious movement out of my head.

Harvey Penick said that most practice swings were pretty useless, as you didn't have to hit anything. Because you weren't swinging at any particular thing or spot, of course it was easy to make a nice smooth swing. But he said this type of swing was only good for rythm and not much else.
His suggestion was, never make another practice swing without swinging at something - a particular blade of grass, a particular spot on the ground, etc. Pick something out and swing at it - this will force you to more replicate your regular swing. And each of these practice swings should hit the spot you are "aiming" at.
Don't know if this'll help you, but it does sound like it's more of a mental thing on your part. Maybe this will do you some good.
