ugly divots

I am having some problems with my divots. I have noticed recently that my divots are aiming to the left, and after further inspection I see that I am digging the toe deep with the heel then opening up and thereby resulting in an open face with a slice. Now this doesn't happen all the time
however, it happens enough to affect my scores and occurs most often on the fairways with my short irons.

Is this a result of:
PATH
ANGLE TOO STEEP
IMPROPER GRIP

if they are going left and oe first my first thought would be and out to in path with a casting or and early release.
