Red lines show the "Width" of the shoulders. Blue the "Hight". The hands should stay within the red, but can go above the blue.

I must be totally mis-understanding this "inside" term
Did the picture Greg provided help? If not, think of it this way:

Take your normal stance with a club in hand. Now lift the arms along with the club up straight up in front of you. Make no turn with the body at this time. You can cock the wrist so the club shaft is pointing up to the sky in front of you. Your hands are now straight out in front of you and between your shoulders. If you keep your arms stiff and just rotate the shoulders to the rear, the hands stay between your shoulders.

Now do the same move as above with your hands out in front of you but instead of turning your shoulders move your hands and arms to the rear, don't rotate your shoulders or hips, try to keep them stationary. You may have to bend your elbows a bit to move the hands back and wrap your arms around your body but your hands have now moved so that they are no longer between your shoulders.

When we make the back swing we are doing both of these motions at the same time. Greg is suggesting to not do the latter, wrapping our arms back around our bodies, so that the hands are no longer between the shoulders. We almost want to raise the hands straight up going back and drop them back down going through.

      Is this right Greg?

        excellent greg !
even I now understand so I need to go off to a mirror and check.
however i suspect my practice swing is good cause i cant get to parallel but my "raeal" swing is maybe too long so maybe I take away with the arms only.
Anyway thanks for your excellent comments and for the time & patience to explain
