I fixed a slight lifting up at the top of my back swing and a poor right arm position as I take the club away with my setup.
MAKE SURE that you can draw a line straight up from your balls of your feet through your knees and back part of your arm...right behind your armpits. This will stop you from lifting up, which is caused by your upper body being in a lazy and bent over position which causes your chin to sag and your left shoulder pushes up on it as you make the backswing. Here is a pic of my setup showing this.

If you have a poor right arm position during the backswing, make sure you have your right elbow pointing at your right hip joint at address, this will insure that your right arm folds properly during the backswing.
These two changes accomplished both situations for me, and I didn't have to 'change my swing' in order for it to happen. A lot of what we do is defined by how we setup and what our angles are at address!
