Your clubface is too open. You are either opening it too much on the backswing or on the downswing. Either way, you are leading with the hosel of the club instead of the sweetspot. You must fix the clubface during the swing, and the earlier the better IMO, by turning the clubface off of the plane. (At the halfway back 90 degree position, try to feel that the clubface is looking away from you). Another way to think of it, is to try and feel that your right palm is facing away from you.
Another feel that helped me if you are opening the club on the downswing (which tends to happen if you are coming too far from the inside because you are trying to hold off that hook-----I am an expert at this BTW) is to feel that the back of you left hand is facing the ground prior to impact. You may need to learn what it feels like for your wrists/forearms to properly release during the swing.
