Re: Putting
You can make a putting guide to groove your stroke. This can be made out of anything - a couple books, some 2X4s, golf balls, tees, etc. Let's use the 2X4s for example. Lay each piece of wood parallel to the target line, just outside the toe and heel of your putter, leaving enough room for your putter to move through. Each piece should be atleast 8" long, with the ball lined up with the middle of the boards.
The purpose of this is to keep your putter from hitting the boards. If you can keep from hitting the boards, this means you have developed and nice, straight back and through swing. This is more difficult than you might imagine because any movment in the hands will cause you to hit a board.

I say it too often: If it's golf club shaped, you can play with it.

Re: Putting
Take your hands out of the stroke completely. Putt with the shoulders. Concentrate on the pendulum action. I found the best way to do this was to lighten the putter, I did this by shortening the putter buy a few inches, With less effective weight you can hold your wrists and hands stiffer.
The board drill is good too. you could also try a chalkline on the putting green.

Re: Putting
Another good drill is this one which will help keep your wrists out of your putts:
Grip the putter with your left hand where you would normally grip with your right hand. Now, with your right hand, grip your left wrist with your right hand so your putter grip in between your left wrist and right hand. It is impossible to use your wrists like this!
