Been there done that, and I'm convinced that in the majority of cases it is a BALANCE issue.
At set up, move ur weight forward towards ur toes until u feel like falling onto ur face.
Hold this position throughout the swing and u should be fine.

Well, I defeated the dreaded sh#@^ks., and this time I actually know what was causing it. It was as you said a weight shifting problem, which in my case was a wobbling head. Bugger up that part of the set up/impact triangle and more than just sh@#$ing starts going wrong.

The shanks are in deed a nasty condition. I sometimes get the infection. Almost knocked someone out at the driving range the other day. I think the golf gods are telling me it's time to get in some serious practice.
